Please take a moment to fill out the new patient forms in the following link. Don’t forget and bring your driver’s license and insurance card(s) to your first appointment. Our forms are HIPAA compliant; your information is protected.
If you are an existing patient and have had any changes to your contact or insurance information, please be sure to let us know at your next appointment so we can get a new copy of your insurance card(s).
Insurance and Payments
At Moore Vision Center, our goal is promote good eye health and enable you to have the clearest vision possible. We accept several types of vision insurance, cash, checks, and most credit cards. We inform our patients about exam and treatment costs at the beginning of your appointment. If you require a breakdown of costs, please let us know.
Payment Options
Moore Vision Center believes in providing the best value for you and your family. We have products and pricing that aims to accommodate all budgets and our office participates in most insurance programs. For patients who have little or no vision insurance coverage, flexible payment programs may be arranged. To learn more about our payment options, please call our office directly.
Our office accepts cash, check, and major debit and credit cards including MasterCard and Visa, as well as Care Credit.
Insurance
Since there are so many plans and coverage varies widely, please call us to find out if your plan covers care in our office. Our staff will verify and explain your benefits before your visit at no charge.

All of the above plans can be used for all your eye care and eyewear needs, including eyeglasses, contact lenses, routine office visits and medical office visits.
Care Credit
